Welcome to Get Back to It! We're excited to work with you and help you get back to doing what you love. As part of the new client process, we require you to complete our intake form. For your convenience, you may complete your intake form ahead of time by completing it below. This form is stored in Healthie, our client record system. Healthie is HIPPA compliant, so you can rest assured your personal information is secure. Completing the form helps us rule out any contraindications that might affect your session and allows us to tailor the session to your unique needs.
